Responsible for the care, well-being, and training for people with intellectual disabilities residing at a residential facility in Florence County. Must be punctual and dependable and have the ability to multi-task in a challenging environment.

Hours: Part-Time / Full-Time shifts available; flexible to work any shift, including weekends
Experience: Minimum 1-year in Human Services, CNA, or related field
Education: High School Diploma and/or degree from an accredited post-secondary school


Day Program Services

Vocational Specialist

Provides training and care to people with intellectual disabilities, performs assigned duties following clearly prescribed standard practices involving the application of readily understood rules and procedures.

Hours: Monday – Friday / 8:00 AM – 3:30 PM; flexible to meet agency needs
Experience: Minimum 1-year in Human Services, CNA, or related field
Education: High School Diploma and/or degree from an accredited post-secondary school


Day Program

Program Manager

Will serve as leader in conducting, tracking, and initiating all individual support plans and plan meetings for coordination; ensuring that the plans are implemented as written and training objectives/goals are recorded on a weekly basis through Therap system. Communicate closely with supervisor and staff to determine progress. Conducts all admissions and discharges and monitors the provisions of Therap by all working closely with supervisors.

Hours: Monday – Friday, 8am – 4pm
Experience: 2+ years in human services or related field
Education: Associate’s degree from accredited school


Day Program

Day Program Director

Will provide oversight and supervise managers on planning and coordinating day operations of the adult day programs county-wide. Responsible for ensuring compliance on safety, documentation and activities on compliance with state/federal regulations for programs related to specific policies and procedures. Monitor and review all general events and incidents through Therap for consistent documentation by managers and staff. Maintain family support throughout service delivery. Responsible for all overall operations in day services and providing training to staff consistently.

Hours: Monday – Friday, 8am – 4pm
Experience: 3+ years in supervisory role working with people with I/DD and special needs environment
Education: Bachelor’s degree in Human Services or related field from accredited school

Quality Assurance Coordinator

Responsible for all ongoing audits and QA initiatives, which include: incident recognition, reporting remediation via corrective actions, training and accountability. Incidents include allegations of abuse, neglect and exploitation and other types of sentinel events or critical incidents which may affect the health, safety, or welfare of those supported. Will provide overall responsibility for incident management in a timely manager, directing reviews, analyzing results and recommendations and follows all plan of corrections. Maintain and provide communication with regulatory agencies to ensure changes in standards, strategic approach, and program compliance. Conduct and evaluate risk management to develop corrective plans and proactive approaches.

Hours: Monday – Friday, 8am – 4pm
Experience: 4+ years in quality assurance working in federal, state or county setting
Education: Bachelor’s Degree from accredited school

Human Services Supervisor I

Supervises over staff of assigned location. Responsible for the care, well-being and training for staff to care for people with intellectual disabilities residing at a residential facility in Florence County.

Hours: Full-time and flexible to work any shift, including weekends
Experience: 2 years’ experience in a supervisory management position within human services field
Education: Associates Degree in Human Services; or HS Degree with experience qualifications above; from accredited schools


Residential Services

PT / FT LPN

Under supervision of Director of Nursing, assist with, and monitor care and treatment of people with developmental disabilities who reside in the community. Perform nursing tasks as assigned, work as part of an interdisciplinary team with knowledge of nursing procedures, sanitation and safety practices.

Hours: Must be flexible to meet the needs of persons served
Experience: 1-year of clinical experience required
Education: LPN Degree from accredited school; Current state board license in good standing
*Must be able to work throughout Florence County


Family Support

Case Manager

Case Managers are assigned a caseload of adult and adolescent clients who have intellectual disabilities, autism, head and spinal cord injuries or other related or similar disabilities. Will be responsible for developing and implementing plans that address all service needs for assigned clients. Plan development involves gathering assessment information from all involved parties, families, clients, and service providers and recording this information as part of a Support Plan. Support Plan implementation then involves authorizing and monitoring services that are deemed to be effective and reflect the client’s personal choices and satisfaction. Must also be available to respond to changing needs and urgent or crisis circumstances for assigned clients as those needs arise.
