Human Services Assistant (HSA)

Responsible for the care, well-being, and training for people with intellectual disabilities residing at a residential facility in Florence County. Must be punctual and dependable and have the ability to multi-task in a challenging environment.

Hours: Part-Time / Full-Time shifts available; flexible to work any shift, including weekends
Experience: Minimum 1-year in Human Services, CNA, or related field
Education: High School Diploma and/or degree from an accredited post-secondary school

PT / FT LPN

Under supervision of Director of Nursing, assist with, and monitor care and treatment of people with developmental disabilities who reside in the community. Perform nursing tasks as assigned, work as part of an interdisciplinary team with knowledge of nursing procedures, sanitation, and safety practices.

Hours: TBD; must be flexible to meet the needs of persons served
Experience: 1-year of clinical experience required
Education: LPN Degree from accredited school; Current state board license in good standing
*Must be able to work throughout lower Florence County

CTH II Coordinator

Under the supervision of the CTH-II Director, the CTH-II Coordinator is responsible for the management of assigned CTH-II facilities, providing direct supervision and training to Human Service Supervisors and Direct Support staff. Developing and updating treatment plans, actively managing the care, welfare, safety, and security of people with developmental disabilities. Ensures electronic records (Therap) and documentation are maintained and managed throughout programs.

Hours: M-F / 8:00 AM – 4:00 PM; also, must be flexible to meet agency needs
Experience: 2 -years of supervisory management in Human Services field
Education: LPN Bachelor’s Degree in Human Services; or related field

Day Program Services

Vocational Specialist

Provides training and care to people with intellectual disabilities, performs assigned duties following clearly prescribed standard practices involving the application of readily understood rules and procedures.

Hours: Monday – Friday / 8am – 3:30pm; flexible to meet agency needs
Experience: Minimum 1-year in Human Services, CNA, or related field
Education: High School Diploma and/or degree from an accredited post-secondary school

Habilitation Manager (IWCC – Lake City)

Manage and coordinate the individual service planning process, interdisciplinary team members, plan development, and plan revisions ensuring
compliance with all documentation. Review and monitor notes are accurate and complete in Therap. Provide staff with ongoing training on standard regulatory requirements by SCDDSN, DHEC, and DOL.

Hours: M-F / 8:00 AM – 4:00 PM; also, must be flexible to meet the needs of persons served
Experience: 1-year of working with people with disabilities and special needs
Education: Associate’s Degree in Human Services from accredited school

Case Management

Case Manager

Case Manager needed to provide services to people with head/spinal cord injuries, intellectual disabilities/related disabilities/autism. Flexibility/afterhours may be required depending on schedule demands. Applicant must be highly efficient and organized, have excellent communication skills, and be able to work independently. Home visits required.

Hours: M-F / 8:00 AM – 4:00 PM; also, must be flexible to meet the needs of persons served
Experience: 1-year of working with people with disabilities and special needs
Education: Bachelor’s or Graduate Degree from accredited college or university
